release: 2.5.1 — codebase_map context mode + quality/CI patch

PATCH release per the versioning policy: a small opt-in additive feature
(`context_mode: codebase_map`) plus the post-2.5.0 quality/CI work — not a
milestone, so the patch digit (not a 2.6.0 jump).

Bumps the version everywhere (pyproject/__init__/test_sdk_surface/CLAUDE),
adds CHANGELOG [2.5.1], and syncs the docs (README banner + repo-state;
README/PLAN_GUIDE/TESTED_GUARANTEES/ROADMAP: 9 -> 10 context modes).
