Avoid mutating submodule __all__ lists

Several package initializers reused mutable __all__ lists imported from
submodules before appending package-level exports. This leaked invalid
names into the submodule API metadata.

Copy the imported lists before extending them, and add a regression test
covering the affected aggregating packages.
