* docs(ai): add instructions for agents

* self review

* docs(ai): clarify provider stance in AGENTS.md

No new in-tree providers are accepted; existing ones are being migrated
out-of-tree (issue #4347). Replace the step-by-step "Adding a New
Provider" section with a note directing agents to the webhook mechanism.
