feat!: remove EnvironmentName option in favor of ResourceAttributes

BREAKING CHANGE: The EnvironmentName option is removed. Set the
environment via ResourceAttributes using the standard
deployment.environment key, e.g.
{ "ResourceAttributes": { "deployment.environment": "production" } }.

Updates the library, tests, sample/test appsettings and README examples.
