test: cover request filter delegate via CreateRequestFilter factory

The inline filter lambda was never invoked in tests (no HTTP request
flows through instrumentation). Extract it into a testable factory and
exercise the delegate with a DefaultHttpContext for both excluded and
non-excluded paths.
