test: extract request filter into testable ShouldInstrument method

Reach 100% line coverage by extracting the AspNetCore filter predicate
into an internal ShouldInstrument method and unit-testing its segment
matching (including the /healthz non-match trap). Also resolve the
signal providers in existing tests so the lazy OTel configuration
lambdas actually execute.
