* fix markdown table formatting: use single pipe in data rows

The correct table format uses `||` for the header row and `|` for
separator and data rows (matching carapace-dev). All other skill
SKILL.md files and several reference files incorrectly used `||`
or `|||` for data rows, breaking table rendering.
