feat(#4): CI4 parity — implicit TLS on 465 and real EHLO hostname

- Port 465 now uses implicit TLS even when encryption is unset (RFC 8314),
  matching CodeIgniter's Email transport. Explicit 'ssl' still forces it on
  any port; 'tls' still means STARTTLS.
- The EHLO argument defaults to gethostname() instead of 'localhost' for
  better deliverability, still overridable via the 'helo' setting.
