feat: --gate enables idle no-activity timeout instead of wall-clock cap (#9)

--gate now enables a 5m idle no-activity timeout measured from the last transcript write, instead of lowering --turn-timeout to a 5m wall-clock cap. A long but continuously-active turn is never killed; only a genuine silent hang trips it. --turn-timeout keeps its 30m default as the wall-clock ceiling.

the stall emits FYA_TRANSIENT_TIMEOUT with terminal_reason "fya_no_activity_timeout" so ralphex retries it as transient; --gate stays accepted so ralphex needs no change. The stall yields to context cancellation and to an already-exited claude (post-exit drain), and respects --idle-timeout=0.
