fix(review-loop): final cleanup pass, addressed 2 findings

- [minor] [app/transcript/tail.go:100] Completion.Eligible now requires
  IdleTimeout > 0, so a --gate run with --idle-timeout=0 (idle completion
  disabled) still catches an eligible-but-hung turn via the no-activity stall
  instead of waiting out the 30m turn timeout
- [test] [app/transcript/events_test.go] add TestCompletionEligible covering
  pending tool, terminal result, post-tool answer, and the idle-disabled case
- [test] [app/turn/runner_test.go] add TestRunnerNoActivityTimeoutFiresWhenIdleDisabled;
  both new tests mutation-verified to fail when the IdleTimeout>0 guard is reverted
