ci: skip-existing on PyPI publish (idempotent re-runs)

A manual re-run of the publish workflow failed with "400 File already
exists" because version 2.4.0 is already on PyPI and PyPI versions are
immutable. Add `skip-existing: true` so re-running the workflow on an
already-published version is a clean no-op; new versions still upload
normally.
