refactor(auth): upgrade otplib from v12 to v13

- Replace authenticator.clone() with TOTP class + explicit plugins
- Convert verifyCode() from sync to async (generate/verify are now async)
- Use epochTolerance instead of window for TOTP time tolerance
- Use toURI() instead of keyuri() for otpauth URI generation
- Add createGuardrails() to allow 10-byte dev secret
- Update all test files to use async TOTP code generation
- All 2474 tests passing
