test: convert balanced tests to Ginkgo

Convert balanced_test.go, balanced_scoring_test.go, and
balanced_adversarial_test.go from standard Go testing to Ginkgo
Describe/Context/It pattern. Matches the project standard.

balanced_test.go moves to package disruption_test (exports only).
balanced_scoring_test.go and balanced_adversarial_test.go stay in
package disruption (internal, access unexported helpers).
