2.30.0: fix four silent QC/config footguns (#167/#168/#170/#172) (#174)

Fixes four silent footgun bugs: dead min_cd3_reads gate (#172, now defaults to 3), silent min_mito_pct floor (#168, default 2.0 kept but now logged/warned), fdr_tiers silently inert under threshold tiering (#170, now warns once), and hardcoded tier1 in method-recovery (#167, now strictest populated tier). Adds unit + behavioral tests for the extracted helpers.
