fix(rateLimit): suppress ERR_ERL_KEY_GEN_IPV6 validation warning

- Add validate: { ipKeyGenerator: false } to all four rateLimit() calls
- Our custom keyGenerator uses req.ip as opaque string key (for Azure ip:port)
- IPv6 normalization is unnecessary for our use-case
