fix: enforce PRIMARY KEY on guilds.id to prevent duplicates

The guilds table was created without a PRIMARY KEY, so INSERT OR REPLACE
degraded to plain INSERT and doubled guilds on every bot restart and
manual refresh. Add idempotent schema migration to both entrypoints
(iDiscord.py + dashboard/main.py) that:
- Detects missing PK on guilds.id via PRAGMA table_info
- Deduplicates existing rows with MIN(name)
- Recreates table with id TEXT PRIMARY KEY
- Prevents future duplicate accumulation
