feat(dashboard): PFP refresh with progress + user cleanup

- Add 'Refresh PFPs' button in Users toolbar
  - Modal confirmation warns this may be slow
  - SSE streaming progress bar shows live count
  - POST /api/users/refresh-avatars/start + SSE /stream
  - Background worker fetches avatars from Discord API,
    sleeps every 50 requests to respect rate limits
  - Re-loads user table on completion

- Add 'Clean Up Users' button (danger style)
  - Modal confirmation warns about permanent deletion
  - POST /api/users/clean-up removes users with no
    birthday (NULL or '00-00') and no guilds
  - Re-loads user table on completion

- Shared reusable .modal, .modal-overlay, .modal-buttons CSS
- Shared showConfirmModal helper in JS
