Repair filesystem block store negative-path spec drift (#31)

* Repair filesystem block store negative-path spec drift

Propagate the filesystem block-store RCA into the spec package and implementation.
Define total constructor/get/put negative-path semantics, add deterministic validation
coverage, and introduce a non-default test injection seam so Windows CI can
exercise the missing failure branches without widening the default runtime API.

- Phase 1/2: update docs/specs/rust-filesystem-block-store/{requirements,design,validation}.md
- Phase 5: update crates/lexongraph-block-store-fs/{Cargo.toml,src/lib.rs,tests/spec_validation.rs}

* Assert integrity-conflict wording in fs store test

Tighten the conflict-path validation so it enforces the spec requirement that
backend failures describe integrity conflict rather than merely matching the
error variant.
