ci: replace markdownlint-cli2 with mado

Switch the Markdown linter to mado (a faster Rust implementation).
Migrate the config to mado.toml, update the lefthook pre-commit hook,
the bin/setup install step, and the CI lint-markdown job accordingly.

Also added space around codeblock in README to satisfy mado's lint checks.
