fix(api): wrap raw errors in connect codes and add missing error logs

Billing authZ interceptor helpers (getOrgForBillingAccount, etc.)
were returning raw Go errors instead of connect.Error, causing
code_0/unknown in logs and metrics. Wrap with proper CodeNotFound,
CodeInvalidArgument, or CodeInternal.

Add error logging to 11 handler code paths that silently swallowed
errors before returning CodeInternal. Fix duplicate request_id and
method fields in the interceptor log.
