Markdown: render inline code on a background fill

Inline code now renders as monospace text on a background fill using the
same colour name as the fenced code block, so inline and block code read
consistently.

Reuses RichTextStyleCodeInline via an unexported marker (mirroring the
existing concealed flag), so no new public API, style or theme constant
is added. A monospace Entry/Label is unaffected because it does not set
the marker.
