fix(document): return 400 when issue field is set on unsupported type

- Add TYPES_ALLOWING_ISSUE constant in config/constants/document.js
- Validate issue field in create and update controllers
- Return 400 with clear message instead of letting DB constraint cause 500
- Add integration tests for both create and update paths
