Provider: Add Appointment confirm button (#9302)

After using `$hold` (such as from the FooMedical example), an
appointment is in the "pending" state.

- Represent this visually with an outlined style instead of the bold
  background color for events in later statuses.

- Add a button to invoke `$confirm` for pending appointments
