feat(monitor): live fleet dashboard (bubbletea) (#35)

- new `taipan monitor` TUI: discovers the fleet (implicit --all), polls each
  device's stats/pool every 5s concurrently, renders a live table
- read-only v1, fleet list only (detail drill-down to follow); requires a tty
- 'r' refresh, 'q' quit; per-device errors render as offline rows
