docs: overhaul README with Mermaid diagrams, update version and test counts

- Replace all Graphviz DOT code blocks with GitHub-renderable Mermaid blocks
- Update install version from ~> 0.5 to ~> 0.8
- Update test count from 310 to 833
- Add Mermaid.js to Themes & Rendering section
- Document all 6 built-in themes
- Check completed roadmap items (centrality, cut vertices, Mermaid)
- Fix stale DecisionTree example for tree invariant compliance
- Remove double --- separator
