feat(reader+demo): lazy facet boot, hide 0-count facets, add boot diagram

Large facet sidecars no longer download their whole postings region at boot: FacetIndex loads the meta region plus only the top-N category heads per field (the tails, which dominate the file and are only needed for filtered tail pagination, are skipped), cutting the 47.8M demo's facet boot from ~231 MB to ~2.4 MB. Small sidecars keep the single-read path. Demo: hide facet categories whose current-query count is 0 (kept when an active filter); add a boot.svg explaining what loads up front vs what stays range-fetched in S3.
