docs: fix unqualified codec:: in README and remove bulk helpers from paper proposal

README.md:
- Qualify codec::utf_8/shift_jis with beman::transcoding:: in three standalone
  snippets (or_error, encode, transcode examples) that lacked using-namespace
  context

papers/transcode-view.md:
- Remove decode_to/encode_to/decode_into/encode_into from the Design Overview
  proposed-API list — they are deliberately not proposed (noted at line 270)
- Replace code examples using those helpers with the equivalent
  view | ranges::to<>() and ranges::copy() patterns
- Fix Roundtrip Tests paragraph that referenced the removed helpers
- Remove decode_to/encode_to from the <transcode> header synopsis
