Make electrodes_file mandatory for LFP reports

Per the spec, each LFP report must specify its own electrodes_file.
Changed from parseOptional to parseMandatory for LFP reports.
Added tests verifying LFP without electrodes_file throws an error.
Pull Request #49: Move electrodes_file to per-report LFP blocks
