Add ADR-0008: cross-credenza federation via peer introspection, update README (ci skip)

  Adds ADR-0008 documenting the federated introspection fallback design for split deployments where tokens issued by one Credenza instance must be validated by another. Covers two gaps: the primary case (DERIVED token issued by Credenza-B is unknown to Credenza-A's introspect endpoint) and the symmetric token-exchange case. Design uses a TRUSTED_INTROSPECTION_PEERS config, RFC 7662 as the peer verification primitive, and a credenza_session_type extension field to prevent transitive exchange of derived sessions. Includes discussion of iss relay semantics, OIDC Federation and GNAP as formal long-term standards, and the AUTH_HOSTNAME workaround as the short-term alternative. Also updates README with links to ADR-0007 and ADR-0008.
