chore: clean up pipeline flow (#34)

## Description

Cleans up the app pipeline flow so prediction outcomes are explicit and
dashboard refresh handling is centralized. Replaces mocked app unit
coverage with integration coverage against Postgres-backed pipeline
behavior.

## Added
- Integration tests for success, skip, and database failure paths.

## Updated
- Refactored app pipeline exit codes, step labels, dashboard refresh,
and Slack alert handling.
- Switched CI dependency install to use the locked environment.

## Deleted
- Removed legacy mocked unit tests and unused logs file setup from
CI/Docker.
