Use double underscore separators for generated names

Adopt the f2py-derived path separator convention for native generated support identifiers as well. Defaults, pads, enum arrays, bounds helpers, runtime dimension locals, candidate variables, partial-set bounds, and f2py bridge temporaries now use double underscores between semantic parts.

Regenerate committed fixtures and examples, update f2py is_set presence flags to has__idx, and document that double underscores are reserved for generated internals. This intentionally does not add new native Fortran identifier length handling.
