docs(repo): fix suggestion ordering instruction to prevent label-first comments

  * invert conditionality: require suggestion fence whenever fix is a concrete change,
    not just "when one exists"
  * add imperative "start typing immediately, nothing comes before it" to eliminate ambiguity
  * remove negative example pattern that could be picked up by the model
