ci: exclude ssh test-helper files from coverage profile

ssh/fakes.go and ssh/testing.go are non-_test.go files that only exist
to support tests (cross-package consumers require their symbols to be
package-visible). Their statements were counting against the coverage
denominator, dragging the total down. Strip their lines from
coverage.out before reporting in both the CI workflow and the local
test-coverage target so Coveralls and `go tool cover` reflect coverage
of production code only.
