docs(contrib): require DCO sign-off on commits

Install the DCO GitHub App as a required PR check. Replace the
'No DCO required' paragraph in CONTRIBUTING.md with a Developer
Certificate of Origin section, document git commit -s, and add a
sign-off checkbox to the PR template.

Closes OpenSSF Best Practices Silver-tier criterion: dco.
