CCDIDASH-76: update studies and samples (#15)

* update studies and samples

* Adjust indexes definition to follow new mappings structure

for both studies_table and samples_table

* Update studies index to count diagnosis unique values

Also adjust pige_size to 1 as study files can be many.

* Improve performance and remove duplicated "_filters" entries
