feat: add plugin structure, CI, and PACKAGE.md support

Plugin:
- .claude-plugin/plugin.json - C3 plugin manifest
- skills/create/SKILL.md - Generate PACKAGE.md for projects

CI:
- .github/workflows/test.yml - Matrix testing (3 OS × 3 Python)
- tox configuration for local multi-version testing
- Coverage reporting via Coveralls

- README.md - Updated with PACKAGE.md explanation and badges
- CLAUDE.md - Project guidance for AI agents

Code quality:
- Type annotations for all functions
- Fixed lint issues in tests
