fix: address PR #34 review comments

- Guard metadatas[i] with bounds check to prevent IndexError
- Fix count_collection() fallback to return real count via query()
  instead of misleading hardcoded 1
- Use get_connection_class('milvus') in provider_factory to go through
  lazy-import path with friendly ImportError message; add default
  values for path and host
- Update README provider matrix: Milvus is experimental, Windows
  support noted as limited due to file-locking issues in Milvus Lite
- Remove unused sys imports from 4 test files
