docs: correct connect_options examples and add release note

The original example and README listed `:pool_timeout` and
`:receive_timeout` as `connect_options`, but those are top-level Req
options and are silently ignored when nested under `:connect_options`.
The `:timeout` key is the socket connect timeout, not a request timeout.

Limit the documented keys to genuine `:connect_options` values, correct
the `:timeout` description, drop the misleading connection-pooling
example, and record the feature in RELEASE.md.
