fix(zellij): route popups to originating tab via pane tracking (#210)

* fix(zellij): route popups to originating tab via pane tracking

- Use `zellij action new-pane --tab-id` to keep popups in the caller's tab
- Detects ZELLIJ_PANE_ID (zellij 0.40+) and resolves to tab_id via `list-panes`
- Filters on `is_plugin=false` to match terminal panes (plugin panes share IDs)
- Falls back to plain `zellij run` if pane ID unavailable or jq missing
- Applies to all three launcher scripts: claude-plugin, codex, and planning

* fix(zellij): simplify popup routing with native --tab-id support

- Replace zellij action new-pane with zellij run --tab-id for direct tab routing
- Add --tab flag to list-panes and filter on tab_id != null for robustness
- Improve error handling with 2>/dev/null and || true fallback
- Apply fix consistently across all three launcher scripts
