Improve passive inline completion hints

Detect keywords expected only as identifiers at the parser level
using a shift-target-state heuristic, and filter them from passive
inline hints. This fixes confusing suggestions like "by" after
"GROUP BY " where BY was only expected as a potential column name.

Also add passiveHint UI state to suppress dropdown list and keyboard
acceptance for between-symbol completions, and rename keyword tiers
from KEYWORD_* to KEYWORD_{A,B,C,D}.
