feat: warn about unused %private functions and variables

Adds scope-aware unused-symbol diagnostics for XQuery modules.
Only %private declarations are reported; public ones may be used by
importers. Names starting with _ are skipped by convention.

- Scope stack tracks local let/for/count/group/window bindings so
  a local $x shadowing a module $x does not suppress the module
  diagnostic
- Function usage keys include arity (#n) to handle overloads
- New ast-nodes.ts helpers: asFunctionDecl, asBinding (polymorphic
  for ForBinding/LetBinding/CountClause/GroupingSpec/WindowClause),
  asPositionalVar, asVarName, asCatchClause, asVariableDeclaration,
  splitName; applied throughout to remove repeated colonIdx boilerplate
