Track local tooling ignore rules and CodeGraph config

Ship the ignore rules and index config for the local dev tooling so they
travel with the repo. .claude/.gitignore keeps the memory MCP server's
plaintext store out of git. .codegraph/.gitignore excludes the per-machine
index database while .codegraph/config.json records the language include
and exclude globs the indexer uses; the 81MB codegraph.db stays untracked
via the nested ignore.
