query: drop max_result_rows +1 trick; cover dynamic-tool row caps with tests

The +1 push to ClickHouse was redundant: with result_overflow_mode='break'
the engine stops between blocks, and realistic block sizes (~64k rows)
deliver well past any sane cap. The Layer 2 overshoot check fires either
way, so the +1 just obscured intent.

Adds two tests for the MaxResultRows path that were missing in #124:

- TestMakeDynamicToolHandler_RowCapTruncates: confirms TruncationInfo and
  the second text-content notice on the MCP dynamic-tool handler.
- TestHandleDynamicToolOpenAPI_RowCapTruncates: confirms X-MCP-Truncated
  header + JSON body on the OpenAPI dynamic-tool path.
