ci: add coverage upload + README badges (test, coveralls, goreportcard)

The test job now produces a Go coverage profile and uploads it via the
official Coveralls action (coverallsapp/github-action@v2 with
format=golang). GITHUB_TOKEN is picked up automatically for the public
repo; Coveralls itself needs the one-time sign-in at coveralls.io to
authorise the repo before the badge starts reporting real numbers.

-covermode=atomic is added alongside -race because the default count
mode is not race-safe with -coverprofile.

README gains three badges at the top: CI status (links to the workflow
run list), Coveralls coverage, and Go Report Card grade.
