Add `Appointment/:id/$confirm` operation (#9210)

* Add `Appointment/:id/$confirm` operation

This operation is invoked for a "pending" appointment created by
`$hold`. It updates the appointment to the "booked" status, and updates
all associated Slot resources to match.
