fix(work): include program relationships on all action and flow fetches

Centralize ACTION_INCLUDE / FLOW_INCLUDE in the entity service and apply
them to every fetch path (single model, byPatient, byFlow) and every
manage:add channel on the dashboard, flow page, and archive. Previously
byPatient, byFlow, and the dashboard/flow/archive manage:add refetches
omitted the program chain, so views could render against a partially
hydrated program model.
