feat: add notifications page deep-link to Slack alerts and GitHub issues

- Add NotificationsURL field to Message and SCMParams, built as
  {uiBase}/notifications?instrument_id={id}&fingerprint={fp}
- Slack Block Kit: add "Manage Silences" button to every alert message
  and to suppression digest messages
- Suppression digests now render as Block Kit (same layout as primary
  alerts) instead of plain text
- GitHub issue body footer: add "Manage silences" hyperlink
