refactor(webui): pool history as collapsible inside Active card (#417)

Pool History was a standalone card sitting after the Pools config rows;
move it inside the Active card as a collapsible disclosure (using the
shared .disclosure utility so the marker matches Diagnostics + the
PoolEditForm Advanced Options expander).

PoolStatsCard tweaks:
- For the active slot, drop the nested box (.flat) and the host:port
  heading — both are redundant with the parent Active card.
- Inline the block count as a fourth grid item alongside shares / hashes
  / best diff instead of a separate badge.
- Drop the "last seen" field — was always "Ns ago" for the active pool
  and didn't carry useful signal.
- Render the best-diff and last-block timestamps as a small sub-line
  under each value (visible by default rather than tooltip-only).

Hero best-diff and blocks tiles get the same session/lifetime relative-
time sub-line, replacing the title="" tooltips.
