docs: fix Alloy pipeline snippet and helix_entity_id note

- helix_entity_id is structured metadata, not a json field — update
  the Loki query hint to use | helix_entity_id = `...` not | json
- Full loki.process pipeline with stage.structured_metadata and
  helix_process_name in stage.labels required for dashboard compatibility
- Add explanation of why each field goes where it does
