* fix: include entity id in GitHub event topics

Use five-segment GitHub topics so subscriptions can filter per pull request through topic patterns alone.

* fix: preserve legacy GitHub event subscriptions

Match five-segment GitHub topics against legacy four-segment subscription patterns during migration and update examples to the new shape.
